In the first place, an estimated 117 million adults have one or more chronic health conditions. As well as, one in four adults have two or more chronic health conditions. In addition, Chronic disease is prevalent among Medicare beneficiaries. Also, most beneficiaries have multiple chronic conditions, which increases the risk for poor health outcomes such as mortality and functional limitations. Moreover, clinicians are missing out on this revenue opportunity. And in turn our patients may be missing an added benefit that providers can offer.
    • What about patient’s that have chronic conditions… Lost revenue that you may be eligible to bill for.
    • Calculate “hidden” times you can include toward the 20-minute requirement. Also, looking closely at 99490.
    • Boost care planning pay up for patients with cognitive impairment (99483).
    • Additionally, identify all care providers that qualify to bill CCM – it might surprise you. Clarity on what providers can quantify to bill.
    • Next, Learn how rural health clinics and FQHCs qualify to bill for CCM services.
    • Also avoid confusion between CCM and transitional care management. There is a difference. 
    • Develop a successful patient-centered care plan document that will withstand an audit every time.
    • Lastly, bullet-proof your CCM documentation to avoid denials and potential audits.
